Dear SRV family,

As you know (just ask your child!), we take sustainability very seriously at The School in Rose Valley. As such, we try to be proactive in our thinking about our imprint on the environment. Instead of mailing paperwork to families, we have posted a series of forms on our web site that you can complete and send electronically directly to our school office. Below is a list of the forms you can access:

• Emergency Card (must complete, print and mail in)
• Grouping Information Form
• Parent Report Form
• Transportation Form
• School Directory Form

In addition, the following documents are required by the state of Pennsylvania:

• All preschool students must send a copy of their immunization record from their pediatrician prior to the start of school.
• All kindergarten through 6th grade students must have a completed copy of the Private Physician's Report.
• A Private Dentist Report is required for all students entering kindergarten and third grade.

The Physician's Report and Private Dentist Report are available on the web site as forms you can print, take to your child's pediatrician or dentist, and then mail to The School in Rose Valley office. Pediatricians and dentists often have their own forms or records they will give to families for school purposes. These forms are acceptable as well.

We will be sending you information about your child's teacher and classroom in August. There will be opportunities to connect with teachers and students/families in your child's class prior to the start of the year. Teachers or classroom parents will contact you directly about these events. In addition, there is a 2008-09 School Calendar on the web site to inform you about school events throughout the school year.
