2016-2017 Authorization & Emergency Form
All students must have an Authorization and Emergency Form on file in the school office. This form is due on/before the first day of school.
2016-2017 Transportation Schedule Form
This form should be filled out for all students in Kindergarten through Oldest Group. Please specify both morning and afternoon schedules, read the form for further details.

Health History for New Students
All parents of new students are required to fill out this form for the school nurse. These forms are confidential and will be kept with the student’s medical records. You can mail or drop this off in the office to Kj.
This form is required for all students entering Kindergarten, 1st and 6th grades. An equivalent form from your physician is also acceptable.
This form is required for all students in Kindergarten and 3rd grade. An equivalent form from your dentist is also acceptable.
Certificate of Immunization
PA Department of Health requires all school-aged children to have immunization records on file with the school nurse. If you choose not to immunize your child(ren) – please fill out the appropriate section of the form stating your reasons for exemption.
Asthma Inhaler Use by Student in School Form
As per Pennsylvania law, parents of school-age children may request their children be allowed to carry and self-administer their asthma inhalers. You and your child’s doctor must fill this out. We recommend, however, that most students’ inhalers be kept in the school office, to be administered by office personnel, only.
Food Allergy Action Form
This form is specifically for students with food allergies or sensitivities.
Record Request Form for New Students
Required for new students who have attended school before. This form is granting permission to your child’s previous school or program to forward his/her records to SRV.
Web Permission Withdrawal Form
The school may post your child’s image and first name on the web (including the school’s public web site, password-protected Parent Portal and Woodshop gallery, Facebook page and YouTube account), unless you submit this form to withdraw your permission.
School Directory Changes Form
The School Directory lists all staff members’ and parents/guardians’ contact information. Unless you fill this out to tell us otherwise, we will list the information you provided in your child’s application for admission. You may also change the information at any time by using this form or contacting the school office.
ListServ Unsubscribe Form
The school will automatically enroll new parents whose names and addresses were listed on your child’s application for admission, unless you use this form to request not to be included. You may also unsubscribe at any time.
